Bupropion (Wellbutrin)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Bupropion

What it is

Bupropion is an antidepressant that works differently from the SSRIs: it blocks the reuptake of dopamine and noradrenaline (an NDRI) and does not act on serotonin. It is used for depression, for seasonal depression, and to help people stop smoking. It is sold as Wellbutrin and, for smoking, Zyban, among other names.

How genes come into it

Bupropion is turned by the liver enzyme CYP2B6 into hydroxybupropion, an active form that reaches higher levels than bupropion itself. People with the slow CYP2B6*6 version form less of it; CYP2C19 also takes part. For stopping smoking, studies have looked at CYP2B6 and at the nicotinic receptor gene CHRNA5, and for depression at dopamine-related genes, with weaker evidence. There is no CPIC or DPWG guideline.

CYP2B6 CYP2B6*1, CYP2B6*4, CYP2B6*5, CYP2B6*6, CYP2B6*18, CYP2B6*22: CYP2B6 *1/*6 + *1/*18 is associated with decreased metabolism of bupropion in people with Tobacco Use Disorder as compared to CYP2B6 *1/*1 + *1/*4 + *1/*5 + *1/*22 + *22/*22.

CYP2B6 CYP2B6*1, CYP2B6*4, CYP2B6*5, CYP2B6*6, CYP2B6*18, CYP2B6*22: CYP2B6 *6/*6 + *6/*18 is associated with decreased metabolism of bupropion in people with Tobacco Use Disorder as compared to CYP2B6 *1/*1 + *1/*4 + *1/*5 + *1/*22 + *22/*22.

CYP2B6 CYP2B6*1, CYP2B6*4, CYP2B6*6: CYP2B6 *1/*4 + *4/*6 are associated with increased metabolism of bupropion in healthy individuals as compared to CYP2B6 *1/*1.

Response (43 findings)

Studies of how well it worked in people carrying a variant.

rs1908557: Allele C is associated with decreased response to bupropion in people with Depression as compared to allele T.

ACE rs8075924: Genotype TT is associated with decreased response to bupropion in people with Depressive Disorder, Major as compared to genotypes CC + CT.

ANKK1, DRD2 rs1800497: Genotypes AA + AG are associated with increased response to bupropion and naltrexone in people with Obesity as compared to genotype GG.

CHRNA5 rs16969968: Genotypes AA + AG are associated with increased response to bupropion, nicotine or varenicline in women with Tobacco Use Disorder as compared to genotype GG.
