Boceprevir (Victrelis)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Boceprevir

What it is

Boceprevir was one of the first direct-acting antivirals for hepatitis C: it blocked the viral NS3/4A protease. Approved in 2011, it was used with peginterferon and ribavirin for genotype 1 hepatitis C and was withdrawn a few years later as better-tolerated, interferon-free medicines replaced it. It was sold as Victrelis.

How genes come into it

Response to boceprevir-based treatment depended strongly on the IFNL3 (IL28B) region: people with the CC genotype at rs12979860 cleared the virus much more often, a finding rated at PharmGKB's highest level. This mostly reflects the interferon part of the treatment. Boceprevir itself was broken down by aldo-keto reductases and CYP3A4. There is no CPIC guideline for boceprevir itself.

Rated evidence (1 rated variant)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

IFNL3;IFNL4 rs12979860: Level 1A · replicated. Efficacy: Chronic hepatitis C virus infection. 5.

Response (9 findings)

Studies of how well it worked in people carrying a variant.

IFNL3, IFNL4 rs12979860: Allele C is associated with increased response to boceprevir, peginterferon alfa-2a, peginterferon alfa-2b, ribavirin or telaprevir in people with Hepatitis C, Chronic as compared to allele T.

IFNL3, IFNL4 rs12979860: Genotype CC is associated with increased response to boceprevir, peginterferon alfa-2a, peginterferon alfa-2b, ribavirin or telaprevir in people with Hepatitis C as compared to genotypes CT + TT.

IFNL4 rs11322783: Allele TT is associated with increased response to boceprevir, peginterferon alfa-2a, peginterferon alfa-2b, ribavirin or telaprevir in people with Hepatitis C, Chronic as compared to allele G.

rs10048158: Allele C is not associated with increased response to boceprevir, peginterferon alfa-2a, peginterferon alfa-2b, ribavirin or telaprevir in people with Hepatitis C, Chronic as compared to allele T.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
