Atomoxetine is a non-stimulant medicine for ADHD (attention deficit hyperactivity disorder) in children and adults. It blocks the reuptake of noradrenaline, which raises its level in the parts of the brain involved in attention and impulse control. It is sold as Strattera, among other names.
Atomoxetine is broken down mainly by the liver enzyme CYP2D6, and the gene has an unusually large effect: people with no working CYP2D6 copies (poor metabolizers) can have around ten times the blood level of people with normal activity, with more side effects such as raised heart rate and blood pressure. CPIC and DPWG have published guidelines on CYP2D6 and atomoxetine, and the US drug label discusses poor metabolizers. A few studies have looked at the noradrenaline transporter gene SLC6A2 and at CYP2C19, with weaker evidence.
