Amoxicillin

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Amoxicillin

What it is

Amoxicillin is a penicillin antibiotic: it blocks the enzymes bacteria use to build their cell wall. It is one of the most prescribed antibiotics, used for ear, sinus, throat, chest, urinary and skin infections, for Lyme disease, and as part of the treatment that clears Helicobacter pylori. With clavulanic acid it is sold as Augmentin; on its own as Amoxil, among other names.

How genes come into it

Amoxicillin is cleared mostly unchanged by the kidneys, so drug-processing genes play little part. Many studies listing it are Helicobacter treatments, where CYP2C19 affects the proton pump inhibitor given alongside, not amoxicillin itself. HLA variants such as HLA-DRB1*15:01 and HLA-B have been linked to liver injury with amoxicillin-clavulanate. There is no CPIC or DPWG guideline.

Rated evidence (2 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

HLA-DQB1 rs9274407: Level 3. Toxicity: Drug Toxicity. 1.

HLA-B HLA-B*18:01: Level 3. Toxicity. 1.

Response (10 findings)

Studies of how well it worked in people carrying a variant.

CYP2C19 CYP2C19 normal metabolizer genotype: CYP2C19 normal metabolizer is associated with decreased response to amoxicillin, clarithromycin and esomeprazole in people with Gastritis as compared to CYP2C19 intermediate metabolizer and poor metabolizer.

CYP2C19 CYP2C19 normal metabolizer genotype: CYP2C19 normal metabolizer is associated with decreased clinical benefit to amoxicillin, clarithromycin and lansoprazole in people with Helicobacter Infections as compared to CYP2C19 poor metabolizer.

CYP2C19 CYP2C19 normal metabolizer genotype: CYP2C19 normal metabolizer is associated with decreased clinical benefit to amoxicillin, clarithromycin and omeprazole in people with Helicobacter Infections as compared to CYP2C19 poor metabolizer.

CYP2C19 CYP2C19*1, CYP2C19*2, CYP2C19*3: CYP2C19 *1/*1 is associated with decreased response to amoxicillin and omeprazole in people with Helicobacter Infections as compared to CYP2C19 *2/*2 + *2/*3.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
