Sufentanil (Sufenta)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Sufentanil

What it is

Sufentanil is a synthetic opioid of the fentanyl family, several times more potent than fentanyl. It is used in anaesthesia and intensive care, in epidurals during labour, and as a tablet under the tongue for severe acute pain in supervised settings. It is sold as Sufenta and Dsuvia, among other names.

How genes come into it

Sufentanil is broken down mainly by CYP3A4. Most studies on it, many from China, looked at how much sufentanil people needed for pain relief after surgery, and linked this to variants in the opioid receptor gene OPRM1 (especially A118G), CYP3A4, ABCB1 and COMT. The results are mixed, and there is no CPIC or DPWG guideline.

Rated evidence (12 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

CYP3A4 rs2242480: Level 3. Dosage: Pain. 6.

OPRM1 rs1799971: Level 3. Dosage: Pain, Pain, Postoperative. 4.

COMT rs4680: Level 3. Dosage. 2.

Metabolism (1 finding)

Studies of how quickly the body processes this medication.

CYP3A4 rs2242480: Genotype T/T is associated with decreased concentrations of sufentanil in women with Pain as compared to genotypes C/C + C/T.

Response (4 findings)

Studies of how well it worked in people carrying a variant.

CYP3A4 rs2242480: Genotype T/T is associated with increased response to sufentanil in people with Pain, Postoperative as compared to genotypes C/C + C/T.

CYP2D6 CYP2D6*1, CYP2D6*10: CYP2D6 *10 is not associated with response to oxycodone or sufentanil in people with Pain, Postoperative as compared to CYP2D6 *1.

CYP3A5 CYP3A5*1, CYP3A5*3: CYP3A5 *3 is not associated with response to oxycodone or sufentanil in people with Pain, Postoperative as compared to CYP3A5 *1.

OPRM1 rs1799971: Genotypes AG + GG are not associated with response to sufentanil in people with Pain, Postoperative as compared to genotype AA.

Dosing (42 findings)

Studies of how much of the drug people needed or tolerated.

ABCB1 rs1045642: Genotype GG is associated with increased dose of sufentanil in children with Pain, Postoperative as compared to genotypes AA + AG.

ABCB1 rs1128503: Genotypes AA + AG are associated with increased dose of sufentanil in people with Lung Neoplasms and Pain, Postoperative as compared to genotype GG.

ABCB1 rs2032582: Genotypes AA + AC are associated with increased dose of sufentanil in people with Lung Neoplasms and Pain, Postoperative as compared to genotype CC.

COMT rs4680: Genotype AA is associated with decreased dose of sufentanil in children as compared to genotypes AG + GG.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
