Sofosbuvir (Sovaldi)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Sofosbuvir

What it is

Sofosbuvir is a direct-acting antiviral for hepatitis C: a nucleotide that the virus's NS5B polymerase builds into its RNA, stopping the chain. Combined with other antivirals (as in Harvoni, Epclusa and Vosevi) it cures most hepatitis C infections in 8 to 12 weeks. It is sold as Sovaldi.

How genes come into it

Sofosbuvir is a prodrug activated inside liver cells by CES1, HINT1 and other enzymes, and studies have looked at variants in these genes for its levels. Others looked at the IFNL3/IFNL4 region, which strongly predicted cure with older interferon treatment; with sofosbuvir-based medicines its effect is small. There is no CPIC or DPWG guideline.

Rated evidence (6 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

IFNL3;IFNL4 rs12979860: Level 3. Efficacy: Chronic hepatitis C virus infection. 2.

IFNL4 rs11322783: Level 3. Efficacy: Chronic hepatitis C virus infection. 1.

IFNL3;IFNL4 rs12979860: Level 3. Efficacy: Chronic hepatitis C virus infection. 1.

IFNL3;IFNL4 rs12979860: Level 3. Efficacy: Chronic hepatitis C virus infection, Recurrence. 1.

Response (9 findings)

Studies of how well it worked in people carrying a variant.

CES1 rs4513095: Allele A is associated with decreased response to sofosbuvir in people with Hepatitis C, Chronic as compared to allele C.

HINT1 rs7728773: Genotypes CT + TT is associated with increased clinical benefit to daclatasvir null sofosbuvir in people with Hepatitis C, Chronic as compared to genotype CC.

IFNL3 rs12979860: Genotype CC is associated with increased clinical benefit to daclatasvir null sofosbuvir in people with Hepatitis C, Chronic as compared to genotypes CT + TT.

IFNL3, IFNL4 rs12979860: Genotype CC is associated with increased response to ledipasvir and sofosbuvir in people with Hepatitis C as compared to genotypes CT + TT.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
