Remifentanil (Ultiva)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Remifentanil

What it is

Remifentanil is an ultra-short-acting synthetic opioid of the fentanyl family. Given as a continuous drip during anaesthesia, it wears off within minutes once stopped, because blood and tissue esterases break it down. It is used during surgery and sometimes for pain relief in labour. It is sold as Ultiva, among other names.

How genes come into it

Remifentanil is broken down by non-specific esterases in the blood and tissues, not by liver CYP enzymes, so the usual drug-processing genes do not apply. Studies have looked at the transporter ABCB1, the opioid receptor gene OPRM1, COMT and KCNJ6 for how much remifentanil people needed and for side effects. The findings come from single or a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (7 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

COMT rs4680: Level 3. Efficacy: Pain. 1.

OPRM1 rs79910351: Level 3. Efficacy. 1.

ABCB1 rs1128503: Level 3. Efficacy, Toxicity: tonsillectomy. 1.

ADRB2 rs1042718: Level 3. Toxicity: Hypotensive disorder. 1.

Response (16 findings)

Studies of how well it worked in people carrying a variant.

ABCB1 rs1045642: Genotype GG is associated with decreased response to remifentanil as compared to genotype AA.

ABCB1 rs1128503: Genotype GG is associated with increased response to remifentanil and sevoflurane in children with tonsillectomy as compared to genotypes AA + AG.

ABCB1 rs1128503: Genotype GG is associated with increased response to propofol and remifentanil in children as compared to genotypes AA + AG.

COMT rs4680: Genotype AA is associated with increased response to remifentanil in infants with Pain as compared to genotypes AG + GG.

Dosing (5 findings)

Studies of how much of the drug people needed or tolerated.

ABCB1 rs1045642: Genotype GG is associated with increased dose of remifentanil as compared to genotype AA.

ADRA2A rs1800544: Genotypes CG + GG is associated with increased dose of remifentanil in women with Pain, Postoperative as compared to genotype CC.

OPRM1 rs1799971: Genotypes AG + GG is associated with increased dose of remifentanil in women with Pain, Postoperative as compared to genotype AA.

OPRM1 rs1799971: Genotypes AG + GG is associated with increased dose of remifentanil in women with surgery as compared to genotype AA.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
