Imidapril (Tanatril)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Imidapril

What it is

Imidapril is an ACE inhibitor: it blocks angiotensin-converting enzyme, so less angiotensin II is made, blood vessels relax and the body holds less salt. It is used for high blood pressure, mainly in Japan and other Asian and European countries; it is not marketed in the US. It is said to cause cough less often than some other ACE inhibitors. It is sold as Tanatril, among other names.

How genes come into it

Imidapril is a prodrug converted into its active form, imidaprilat, by liver esterases. The few gene studies on it looked at CYP11B2 (aldosterone synthase) and AGT for blood pressure response, and at the bradykinin receptor gene BDKRB2 for cough. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (3 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

CYP11B2 rs1799998: Level 3. Efficacy: Essential hypertension. 1.

BDKRB2 rs1799722: Level 3. Toxicity: Cough, Essential hypertension. 1.

AGT rs5051: Level 3. Efficacy: Hypertension. 1.

Response (4 findings)

Studies of how well it worked in people carrying a variant.

AGT rs5051: Genotype CC is associated with increased response to benazepril or imidapril in people with Hypertension as compared to genotypes CT + TT.

CYP11B2 rs1799998: Genotypes AA + AG are associated with increased response to benazepril or imidapril in people with Essential hypertension as compared to genotype GG.

KCNH2 rs1137617: Genotype GG is not associated with response to candesartan, imidapril or irbesartan in people with Essential hypertension as compared to genotypes AA + AG.

CYP11B2 rs1799998: Genotypes AA + AG are not associated with response to benazepril or imidapril in people with Essential hypertension as compared to genotype GG.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
