Ibrutinib (Imbruvica)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Ibrutinib

What it is

Ibrutinib is a Bruton's tyrosine kinase (BTK) inhibitor that blocks a key signal B cells need to survive and multiply. Taken once daily, it is used for chronic lymphocytic leukaemia, mantle cell lymphoma, Waldenström's macroglobulinaemia and chronic graft-versus-host disease. Atrial fibrillation and bleeding are known side effects. It is sold as Imbruvica.

How genes come into it

Ibrutinib is broken down almost entirely by CYP3A4, so CYP3A4 blockers raise its levels many-fold. The gene studies listed looked at ABCG2 and CYP2D6 for its levels. Resistance arises from acquired mutations in BTK itself in the cancer cells, not from inherited genes. There is no CPIC or DPWG guideline.

Metabolism (2 findings)

Studies of how quickly the body processes this medication.

ABCG2 rs2231142: Genotypes GT + TT is associated with increased clearance of ibrutinib in healthy individuals as compared to genotype GG.

CYP2D6 rs1065852: Genotypes AA + AG is associated with decreased clearance of ibrutinib in healthy individuals as compared to genotype GG.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
