Glucarpidase is a bacterial enzyme (carboxypeptidase G2) that rapidly breaks down methotrexate in the blood into inactive products. It is given by injection as an antidote when high-dose methotrexate builds up to toxic levels because the kidneys are not clearing it fast enough. It is sold as Voraxaze.
As a protein, glucarpidase is not handled by drug-processing enzymes. The gene studies listed concern methotrexate clearance in children with leukaemia, the situation in which glucarpidase is needed: SLCO1B1, ABCC4, ARID5B and others were linked to slower methotrexate clearance and toxicity. The findings come from a few studies, and there is no CPIC or DPWG guideline.
