Dolutegravir (Tivicay)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Dolutegravir

What it is

Dolutegravir is an HIV medicine of the integrase inhibitor group: it blocks integrase, the enzyme HIV uses to insert its genetic material into the cell's DNA. It is a preferred part of first-line HIV treatment worldwide because it is potent and the virus rarely becomes resistant to it. It is sold as Tivicay and in combination tablets such as Triumeq, Dovato and Juluca.

How genes come into it

Dolutegravir is cleared mainly by UGT1A1, with help from CYP3A4. People with low-activity UGT1A1 versions (such as *28/*28, the Gilbert's syndrome genotype) have somewhat higher levels, which the US FDA lists as an effect on drug levels, and studies have looked at whether this relates to sleep and nervous system side effects. Studies have also looked at ABCG2 and NR1I2. There is no CPIC or DPWG guideline.

Published guidelines

Clinical guidelines that name a gene for Dolutegravir. They sort people into groups by the gene copies they carry.

US FDA: UGT1A1. Poor Metabolizer. Data that Demonstrate a Potential Impact on Pharmacokinetic Properties Only.

Rated evidence (2 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

ABCG2 rs2231142: Level 3. Metabolism/PK: HIV infectious disease. 1.

NR1I2 rs2472677: Level 3. Toxicity: adverse events, Discontinuation. 1.

Metabolism (15 findings)

Studies of how quickly the body processes this medication.

ABCG2 rs2231142: Genotype TT is associated with increased concentrations of dolutegravir in people with HIV Infections as compared to genotypes GG + GT.

ABCG2 rs2231142: Genotype TT is associated with increased concentrations of dolutegravir in people with HIV Infections as compared to genotypes GG + GT.

NR1I2 rs2472677: Genotype TT is associated with increased concentrations of dolutegravir in people with HIV Infections as compared to genotypes CC + CT.

UGT1A1 UGT1A1*1, UGT1A1*28, UGT1A1*37: UGT1A1 *28/*28 + *28/*37 (assigned as poor metabolizer phenotype) is associated with increased exposure to dolutegravir in people with HIV infectious disease as compared to UGT1A1 *1/*1 (assigned as normal metabolizer phenotype) .

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
