Bumetanide (Bumex)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Bumetanide

What it is

Bumetanide is a loop diuretic, about 40 times more potent than furosemide by weight: it blocks salt reabsorption in the loop of Henle, causing a strong increase in urine. It is used to clear fluid build-up in heart failure, kidney disease and liver cirrhosis. It is sold as Bumex and Burinex, among other names.

How genes come into it

Bumetanide is cleared partly unchanged by the kidneys and partly by the liver. The gene studies on it, mostly from one study of loop diuretics, looked at genes of salt handling and blood pressure (NPPA-AS1, ADD1, SLC12A3, GNB3, ACE) for how much urine and salt the kidneys passed. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (5 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

SLC12A3 rs1529927: Level 3. Efficacy. 2.

GNB3 rs5443: Level 3. Efficacy. 1.

NPPA-AS1 rs5063: Level 3. Efficacy. 1.

ADD1 rs4961: Level 3. Efficacy. 1.

Response (7 findings)

Studies of how well it worked in people carrying a variant.

ACE rs1799752: Genotype del/del is associated with increased response to bumetanide, furosemide and torasemide in healthy individuals as compared to genotype ATACAGTCACTTTTTTTTTTTTTTTGAGACGGAGTCTCGCTCTGTCGCCC/ATACAGTCACTTTTTTTTTTTTTTTGAGACGGAGTCTCGCTCTGTCGCCC.

ADD1 rs4961: Genotype TT is associated with increased response to bumetanide, furosemide and torasemide in healthy individuals as compared to genotype GG.

GNB3 rs5443: Genotype TT is associated with decreased response to bumetanide, furosemide and torasemide in healthy individuals as compared to genotype CC.

NPPA-AS1 rs5063: Genotype TT is associated with decreased response to bumetanide, furosemide and torasemide in healthy individuals as compared to genotype CC.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
