Benazepril (Lotensin)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Benazepril

What it is

Benazepril is an ACE inhibitor: it blocks angiotensin-converting enzyme, so less angiotensin II is made, blood vessels relax and the body holds less salt. It is used for high blood pressure and, often combined with amlodipine (Lotrel), to lower cardiovascular risk. It is sold as Lotensin, among other names.

How genes come into it

Benazepril is a prodrug turned into its active form benazeprilat by liver esterases, not by CYP enzymes. Most gene studies on it, many from China, looked at genes of the renin-angiotensin system (AGT, ACE, AGTR1, CYP11B2) and folate genes (MTHFR, MTR) for how much blood pressure fell. The findings come from single or a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (8 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

ACE rs1799752: Level 3. Efficacy: Diabetes Mellitus. 2.

MTHFR rs1801133: Level 3. Efficacy: Hypertension. 2.

CYP11B2 rs1799998: Level 3. Efficacy: Essential hypertension. 1.

PRCP rs2229437: Level 3. Efficacy: Hypertension. 1.

Response (25 findings)

Studies of how well it worked in people carrying a variant.

ACE rs1799752: Genotype del/del is associated with increased response to benazepril in people with Essential hypertension as compared to genotypes ATACAGTCACTTTTTTTTTTTTTTTGAGACGGAGTCTCGCTCTGTCGCCC/ATACAGTCACTTTTTTTTTTTTTTTGAGACGGAGTCTCGCTCTGTCGCCC + ATACAGTCACTTTTTTTTTTTTTTTGAGACGGAGTCTCGCTCTGTCGCCC/del.

ACE rs1799752: Genotype del/del is associated with increased response to benazepril or perindopril in people with Diabetes Mellitus as compared to genotypes ATACAGTCACTTTTTTTTTTTTTTTGAGACGGAGTCTCGCTCTGTCGCCC/ATACAGTCACTTTTTTTTTTTTTTTGAGACGGAGTCTCGCTCTGTCGCCC + ATACAGTCACTTTTTTTTTTTTTTTGAGACGGAGTCTCGCTCTGTCGCCC/del.

ADRB2 rs1042713: Genotypes AG + GG are associated with increased response to benazepril in people with Essential hypertension as compared to genotype AA.

AGT rs4762: Allele G is associated with increased response to benazepril in people with Hypertension.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
