About Arsenic Compounds

What it is

Arsenic compounds used as medicines centre on arsenic trioxide, which, together with all-trans retinoic acid, has made acute promyelocytic leukaemia (APL) one of the most curable leukaemias. It makes the abnormal PML-RARA fusion protein that drives APL break down. It is given by drip and sold as Trisenox. This page collects studies on arsenic compounds as a group.

How genes come into it

In the body, arsenic is methylated by the enzyme AS3MT, and people differ in how fast they do this. The few gene studies listed looked at AS3MT and at CBS, a gene of the methionine cycle, for arsenic side effects. The findings come from a few studies, and there is no CPIC or DPWG guideline. Whether arsenic trioxide works depends on the PML-RARA fusion in the leukaemia cells, which is acquired, not inherited.

Side effects (3 findings)

Studies of side effects in people carrying a variant.

AS3MT rs11191439: Allele C is associated with increased metabolism of Arsenic compounds in healthy individuals as compared to allele T.

CBS rs234709: Genotypes CT + TT are associated with increased metabolism of Arsenic compounds as compared to genotype CC.

CBS rs4920037: Genotypes AA + AG are associated with increased metabolism of Arsenic compounds as compared to genotype GG.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
