Abiraterone (Zytiga)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Abiraterone

What it is

Abiraterone blocks CYP17A1, an enzyme needed to make androgens, so testosterone production falls in the testes, adrenal glands and the tumour itself. It is given with prednisone for advanced prostate cancer, both hormone-sensitive and castration-resistant. It is sold as Zytiga and Yonsa.

How genes come into it

Studies have looked at genes of the androgen pathway for how well abiraterone worked: SRD5A2 and HSD3B1 (its 1245C variant, which lets tumours make more potent androgens), CYP17A1, its target, and the transporter SLCO2B1, which carries it into cells. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (2 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

DSE;TSPYL1 rs3828743: Level 3. Efficacy: Prostatic Neoplasms. 1.

SRD5A2 rs523349: Level 3. Efficacy: Prostatic Neoplasms. 1.

Metabolism (5 findings)

Studies of how quickly the body processes this medication.

HSD3B1 rs1047303: Genotype AC is associated with increased metabolism of abiraterone in men with Prostatic Neoplasms as compared to genotype AA.

SLCO2B1 rs1077858: Genotypes AA + AG is associated with increased concentrations of abiraterone in healthy individuals as compared to genotype GG.

SRD5A2 rs523349: Genotype CC is associated with increased metabolism of abiraterone in men with Prostatic Neoplasms as compared to genotypes CG + GG.

HSD3B1 rs1047303: Genotype AC is not associated with metabolism of abiraterone in men with Prostatic Neoplasms as compared to genotype AA.

Response (8 findings)

Studies of how well it worked in people carrying a variant.

HSD3B1 rs1047303: Genotype AC is associated with decreased response to abiraterone and dutasteride in men with Prostatic Neoplasms as compared to genotype AA.

SRD5A2 rs523349: Genotype CC is associated with increased response to abiraterone in men with Prostatic Neoplasms as compared to genotypes CG + GG.

TSPYL1 rs3828743: Allele A is associated with decreased response to abiraterone and prednisolone in people with Prostatic Neoplasms as compared to allele G.

YBX1 rs10493112: Genotypes AC + CC is associated with increased clinical benefit to abiraterone in men with Prostatic Neoplasms as compared to genotype AA.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
